BMI vs Body Fat — Which Measurement Is Most Accurate?

BMI (Body Mass Index) is easy to calculate but only measures weight in relation to height — not how weight is distributed. Body fat measures the actual proportion of fat in the body and gives a more complete picture. Both have their uses, but they answer different questions.

What does BMI measure?

BMI is calculated as weight (kg) divided by height² (m²). The result is placed in one of four categories: underweight (below 18.5), normal weight (18.5–24.9), overweight (25–29.9) or obese (30+).

The advantage of BMI is its simplicity — you only need weight and height. The disadvantage is that it doesn't distinguish between muscle mass and fat tissue. A well-trained person with high muscle mass can end up in the "overweight" category according to BMI despite low body fat percentage.

What does body fat measure?

Body fat is the proportion of your body weight that consists of fat tissue. It is measured with methods such as the US Navy method (based on body measurements), skinfold measurement, bioimpedance or DEXA (X-ray). DEXA is the gold standard but requires specialist equipment.

Body fat gives a more precise picture of body composition — how much is muscle, fat, bone and fluid. This is particularly important for people with high muscle mass (athletes, powerlifters) where BMI systematically overestimates health risk.

When BMI is sufficient — and when it misses

BMI works as a rough screening tool at the population level and for most normal-weight people without extremely high or low muscle mass. It's quick, free and requires no equipment.

BMI is however misleading for:

  • Muscular individuals (trained, athletes, powerlifters)
  • Older people with low muscle mass but normal weight ("sarcopenic obesity")
  • Pregnant women
  • Children and adolescents (require age-specific reference values)
  • Individuals with very low muscle mass and high body fat percentage ("skinny fat")

Which goal should you use?

Which measure is better — BMI or body fat?
Body fat is more informative for most purposes. BMI can misjudge health in muscular individuals (classified as overweight), older people (low muscle mass gives normal BMI despite high body fat) and shorter people. Body fat directly addresses what actually matters for health risk.
Can you have a normal BMI but high body fat?
Yes — this is called 'normal-weight obesity' or 'skinny fat'. It's relatively common in people with little muscle mass. A person at 170 cm and 65 kg has BMI 22.5 (normal weight) but can have 30% body fat if they have little muscle mass.
Can you have a high BMI but low body fat?
Yes — muscular individuals are often classified as 'overweight' on the BMI scale despite low body fat and excellent health. A well-trained person at 175 cm and 85 kg (BMI 27.8) can have 12% body fat and be in excellent shape.
Which measure should you use to set a calorie goal?
Neither BMI nor body fat alone. TDEE (your actual calorie needs) is the starting point for calorie planning. Body fat is valuable information for understanding where you are and where you want to go — but the calorie goal is set against TDEE, not against a body fat percentage.
